GENERAL SUMMARY:

Provides leadership to the development, implementation, monitoring and ongoing improvement of the network's clinical integration program. Identifies and facilitates advancement of clinical system redesign and reengineering initiatives that drive alignment toward system transformation from volume to value, reduce duplication of efforts, and maximize financial performance under risk contracts.

 

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Works closely with network and hospital leadership and other partner organizations to drive development and execution of cross-continuum strategies and measurement systems that improve Quadruple Aim goals of population health, experience of care, cost of care and provider satisfaction
  • Develops a cross-continuum perspective by building relationships, understanding influences, and facilitating shared agendas across the acute, post-acute and ambulatory clinical network, community health providers, and health and social service agencies and collaborative.
  • Facilitates integration between activities of key business units related to clinical transformation that creates alignment, reduces duplication of efforts and improves coordination of approaches and deployment of resources.
  • Provides leadership to network staff, Committees and workgroups around clinical program development and implementation. Ensures decisions are based on high quality clinical evidence and performance data.
  • Provides leadership for performance analysts and IT specialists assigned to the development and implementation of standardized documentation, analytic and reporting systems for clinical and payer claims data to support prioritization and evaluation of clinical improvement programs
  • Facilitates development and implementation of cross-continuum collaborations, projects and work teams that support shared value propositions/business plans
  • Supports the development, implementation and monitoring of strategies to align and prioritize action toward established goals and milestones that demonstrate improvements in quality, service and cost reduction through the redesign of people, processes, information and technology
  • Provides leadership oversight for development and maintenance of dashboards and action plans ensuring metrics and cascading 30 day work plans are monitored and used regularly for performance improvement
  • Leads teams; assesses and monitor workload, administrative and support systems, and internal reporting relationships; identifies opportunities for improvement and directs the implementation of changes.

 

EDUCATION/EXPERIENCE REQUIRED:

  • Master's degree in Health Care Administration, Nursing or related field.
  • Five (5) years of experience in healthcare, managed care or clinically integrated network setting with at least three (3) years of experience in leadership of healthcare operations or systems redesign.
  • Demonstrated strong operational and project leadership skills including developing and implementing systems to hardwire processes, drive accountability, delegate and communicate performance expectations and apply prioritization and proactive problem-solving to ensure achievement of deliverables and outcomes.
  • Demonstrated knowledge and experience applying clinical integration best practice approaches to systems redesign and care transformation.
  • Experience implementing a variety of improvement methodologies including development and management of performance dashboards with cascading metrics and aligned action plans.
  • Management skills necessary to oversee operations, budgets, and staffing/ supervision.
  • Demonstrated effectiveness working in a complex and rapidly changing industry with multiple entities and constituencies.
  • Demonstrated leadership ability, communications, and interpersonal skills necessary to work effectively with physicians, management, staff, external agencies/customers and patients/families.

 

CERTIFICATIONS/LICENSURES:

  • Project Management Professional Certification preferred.
