GENERAL SUMMARY

 Under the supervision of the Quality Improvement Clinical Manager, utilizes knowledge and application of nursing and/or therapy principles to review clinical documentation to ensure accreditation, regulatory, and reimbursement compliance and to promote quality outcomes. Implements Home Health Care quality improvement programs. May provide direct care to patients in the home. Performs other related duties and projects as assigned. 

The individual hired for this role will be responsible for teaching new & established clinical staff all aspects of home health care documentation with a focus on OASIS accuracy.  He/she will coordinate with the Branch and Education department to provide new clinical staff with a comprehensive orientation. Additional responsibilities include teaching Oasis and other classes, working with the Quality department to meet CMS requirements, and supporting the orientee/team/Quality department however necessary.Candidates must have good communication and public speaking skills, attention to detail and flexibility.  This position requires the person to have at least one year of home care experience with preferred precepting, education and/or quality experience & demonstrate a high level of current accuracy of home care documentation.   Direct patient care to patients in the home may be required, when needed. He/she will be based at Henry Ford’s Home Health Administration Office at One Ford Place but may travel to all three home health care branches to provide support to orientees. 

EDUCATION/EXPERIENCE REQUIRED

Requires one of the following: Registered Nurse - Associates Degree or Bachelor's Degree from an Accredited School of Nursing; Physical Therapist - Bachelor's Degree (Bachelor's degree with 5 years of experience OR Master's/Doctorate degree acquired from a Commission on Accreditation of Physical Therapy (CAPTE) accredited program.); Occupational Therapist - Bachelor's Degree (From an Accreditation Council for Occupational Therapy Education (ACOTE) accredited program); Speech Language Pathologist - Master's Degree (Master's Degree in Speech Language Pathology from an accredited graduate program; successful completion of the National Praxis Examination; Successful completion of the Clinical Fellowship year.). Minimum of one year home health care experience required; quality improvement experience preferred. Strong interpersonal and communication skills required to interact effectively with individuals from various cultural, educational, and socio-economic backgrounds. Must be able to lift, carry, transport, and move patients if needed. Manual dexterity for independent direct patient care and use of office equipment required. Must have reliable transportation and a telephone.

CERTIFICATIONS/LICENSURES REQUIRED

 Current licensure by the State of Michigan as a therapist or registered nurse. Must possess a current State of Michigan driver's license and a satisfactory driving record.
