The Master's-level Social Worker (MSW) plays a vital role within Henry Ford Jackson's palliative care team, providing essential psychosocial support and care coordination for patients and families facing serious illness. This position requires a Master of Social Work degree and offers a dynamic practice environment, splitting time between our inpatient palliative care unit and community-based home visits.
The social worker conducts comprehensive psychosocial assessments, facilitates difficult conversations about goals of care, and provides counseling support to help patients and families navigate the emotional challenges that accompany life-limiting diagnoses. Key responsibilities include connecting families with community resources, coordinating discharge planning, addressing barriers to care, and advocating for patients' wishes and values throughout their care journey.
Working collaboratively with physicians, nurses, chaplains, and other team members, the MSW ensures that the social and emotional aspects of care are seamlessly integrated with medical treatment. GENERAL SUMMARY:
Under general supervision, the Social Worker in Palliative Care is responsible for implementing treatment programs to assist homebound clients and their families to adjust to personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefits from medical care.
EDUCATION/EXPERIENCE REQUIRED:
- Master's degree in social work required.
EXPERIENCE REQUIRED:
- Requires a minimum of one (1) year of social work experience in a health care setting.
- Knowledge of the principles of human behavior, individual and group counseling techniques and relevant statutes and regulations.
- Requires very high levels of social sensitivity, personal emotional adjustment and verbal skills in order to provide counseling, assist, and influence others.
- Requires a professional level of analytical skills in order to solve problems, analyze, and develop policies and procedures.
CERTIFICATIONS/LICENSURES REQUIRED:
- Requires Licensure in the State of Michigan as a Social Worker (LMSW).
- Must possess a current State of Michigan driver's license and a satisfactory driving record.
