EDUCATION/EXPERIENCE REQUIRED:
Graduate of an accredited school of nursing
Current enrollment in a Bachelor’s in Nursing (BSN) program or active pursuit of BSN degree with the completion of six (6) semester hours during the first year of employment. Must remain enrolled in BSN program and complete six (6) semester hours annually until degree is obtained
Discharge planning experience
Working knowledge/experience in utilization management, managed care, and payer issues
Supports Jackson Health Network’s mission and goals
Effective verbal and written communication skills along with exceptional computer and critical thinking skills – to be used in all aspects of care
Demonstrates leadership ability, and interpersonal/customer service skills necessary to interact effectively with physicians, management, staff, external agencies/customers and patients/families is required
Knowledge of care of neonate, pediatric, adolescent, adult, and geriatric patients is required
Establish priorities and coordinate work activities among multiple requests
Knowledge about the care of pediatric, adolescent, adult and geriatric patients

CERTIFICATIONS/LICENSURES REQUIRED:
License to practice as a Registered Nurse in the State of Michigan

PREFERRED QUALIFICATIONS:
Bachelor’s Degree in Nursing (BSN)
Minimum of three years’ experience working with patients in ambulatory or community health setting
Experience in chronic care management and/or case management, and primary care is desirable
An understanding on community resources and how to apply them to a care plan
