Registered Nurse- Home Infusion Therapy
RESPONSIBILITIES:
- Under the direction of the Nurse Manager, works to ensure accurate and appropriate gathering of demographic and clinical patient information prior to discharge from the hospital and start of care.
- Assess patients for appropriateness of home infusion therapy.
- Peripheral IV Insertion and administration of specialty IV therapies as ordered, within the patient’s home
- Obtains consent and performs patient and family education prior to start of care when applicable.
- Works with referral sources to identify problems and resolutions.
- Assists in preparing documents that are necessary for billing function.
- Responsible for educating, developing and strengthening relationships with referral sources.
- Assists with research, record keeping, scheduling, communicating, and educating home care agency nurses.
- Coordinates and assists Hospital Case Managers in insuring a smooth transition for patients from the hospital and physician offices.
EDUCATION/EXPERIENCE:
- Graduation from an accredited school of nursing.
- At least one year of experience as a Registered Nurse with peripheral IV Insertion and administration
- Patient education experience preferred.
- A valid drivers' license and an acceptable driving record.
- Shares on-call responsibilities.
- Skills must be proficient in word processing, writing, communication, critical thinking, and technical competency.
- Current licensure as a Registered Nurse in the State of Michigan.
