GENERAL SUMMARY: 
Provides individualized diabetes self-management education/ training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Certified Diabetes Educator. Instructs patients/family members in the Diabetes Self-Management Education/Training (DSME/T) programs associated with Henry Ford Health System. For patients with diabetes provides other clinicians and support staff direction and support in the provision of care. Participates in the development and implementation of innovations in care delivery for diabetes programs within the Population Health Management Unit. 

PRINCIPAL DUTIES AND RESPONSIBILITIES: 
Clinical/Education Activities: 
  • Demonstrates advanced knowledge and skills in the treatment and evaluation of diabetes. 
  • Collects and documents in electronic medical record DSME/T program participant’s assessment, educational plan with goals, and services provided with evaluation of results in a collaborative with patient, provider, and care giver in ongoing manner. 
  • Assist in developing DSME/T curriculum as directed. 
  • May provide Medical Nutritional Therapy (MNT) services to patients and/or families including assessment of nutritional status, nutrition care planning, counseling and evaluation. (for Dietitians in the role) 
  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods. 
  • Utilizes professional judgment and critical thinking to assist patients in overcoming barriers to goal achievement. Works with external agencies in obtaining assistance as necessary. 
  • Evaluates effectiveness of educational services provided to patients. 
  • Conducts diabetes classes and individual encounters. 
  • Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures. 
  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed. Utilizes expertise and specialization to provide consultation to physicians, healthcare providers and consumers. Serves as a clinical resource for nursing and medical staff. Works collaboratively with the multidisciplinary education and treatment teams.
  • Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs. Consults with medical and support staff concerning patient needs and status to provide an optimal educational experience and to assist in determining the plan of care. 
  • Participates in the development of training materials. 
  • Contributes to, and participates in, a continuous quality improvement process to measure DSME/T program and to identify and address opportunities for improvement. 
  • Appraises his/her performance to identify areas of strength and areas for improvement and to develop a plan for improvement and growth. 
  • Maintains current knowledge and skills in specialty area of diabetes and chronic disease care. Participates in research studies/pilot programs as appropriate.
  • Participates in peer review process to evaluate performance of other professional instructional staff. 
  • Participate in relevant programs associated with System-wide Chronic Care Initiatives including assisting with the design and development and delivery of innovative models of patient care intended to improve patient outcomes and to improve the quality, safety, efficiency and effectiveness of chronic care delivery. 
  • Communication/interpersonal skills with patients, co-workers, and leadership at varying levels within the organization. Listening and conflict resolution skills are exercised as appropriate. 
  • Openness to new ideas/innovations, flexibility in approach/thought-process, confidence with decision-making responsibilities 
  • Performs all job duties and responsibilities in a courteous and customer-focused manner. 
  • Performs other related duties as assigned.

EDUCATION/EXPERIENCE REQUIRED: 
  • In-depth knowledge about current diabetes treatment management. 
  • A healthcare professional (Nurse, Dietitian, Pharmacist or Social Worker) or who is also a certified diabetes educator, or who is eligible to become a certified diabetes educator. 
  • Possession of/or actively working to achieve Certification by the National Certification Board for Diabetes Educators (CDE) required; must be achieved within 2 years of hire date. 
  • Minimum (2) two years of experience required in healthcare profession eligible to become a certified diabetes educator. 
  • Excellent verbal communication and written documentation skills. 
  • Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization. 
  • CDE preferred. 
  • Bachelor’s degree in healthcare profession eligible to become a certified diabetes educator preferred. 
  • For Dietitians, Bachelor’s degree in Dietetics required. 
  • Able to lead and effectively manage groups. 
  • Knowledge of clinical practice guidelines, behavior change theory, motivational interviewing and Medicare and Medicaid regulations. 
  • Understanding of community resources and the needs of diverse populations.
  • Creative and resourceful with good negotiating skills preferred. 
  • Computer skills and knowledge including Windows and Microsoft Word, Excel and PowerPoint software applications preferred.
CERTIFICATIONS/LICENSURES REQUIRED: 
  • Dietitians must be registered with the Commission on Dietetic Registration 
  • Dietitians must meet the requirements of the Academy of Nutrition and Dietetics 
OR 
  • Current RN license in the State of Michigan. 
OR 
  • Current Pharmacy license in the State of Michigan 
OR 
  • Current Social Worker license in the State of Michigan
