Post-Acute Network Program Manager (Hybrid - Troy, MI) - Populance
EDUCATION/EXPERIENCE REQUIRED:
- Bachelor’s degree in healthcare related field such as BSW, BSN, health management/administration, Project Management, and/or business administration
degree. - Minimum 3-5 years of healthcare related experience in healthcare delivery, project management, and running large projects of a complex nature, required.
- Minimum three (3) years of experience in project management or analytics-oriented roles, focused on delivering strategic insights to customers/clients.
- Two (2) years of experience using data to drive business decisions, preferred.
- Strong knowledge of Microsoft Word, Excel, Power Point, Project Management.
- Previous clinical experience delivering patient care preferred.
- Excellent verbal communication and written documentation skills.
- Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization.
- Strong problem-solving, analytical, and decision-making skills.
- Strong organizational, planning and implementation skills.
- Strong presentation skills.
- Strong sense of compassion with the ability to successfully advocate for patients and their families.
CERTIFICATIONS/LICENSURES REQUIRED:
- Project Management Institute Certification, preferred.
PHYSICAL DEMANDS/WORKING CONDITIONS:
- Requires 80% local travel to post-acute care facilities within the market area.
- Unpredictable environment with wide variations in the types of patients and acuity levels, varying conditions regarding temperature control and potential exposure to hazardous materials.
- Normal hazards associated with healthcare environment.
