DUTIES/RESPONSIBILITIES:

Assist with the following:

  1. Consult with at-risk patients in High Risk Antepartum, Labor and Delivery and NICU at HFWB and HFH to help process traumatic events such as long-term admissions, emergency cesarean deliveries, postpartum hemorrhage, severe preeclampsia, infant admissions, poor outcome, or loss.
  2. Ensure current evidence based-practice and clinical assessment protocols are being utilized by clinicians across the system
  3. Providing information to clinicians and patients regarding available mental health and support services within the community
  4. Implement ROSES as a virtual education and support tool, available to patients across the system
  5. Implement a supportive group or program for pregnancy and infant loss
  6. Creating a packet of tools and resources for new parents to take home upon discharge.
  7. Maintain contact with at risk patients during pregnancy and up to one year postpartum.
  8. Facilitate multidisciplinary care planning for at-risk perinatal patients or families with a poor prognosis (e.g. Trisomy 18, cardiac anomalies, PPROM)
