Perinatal Outreach Support & Treatment (Post) Network Community Liaison
DUTIES/RESPONSIBILITIES:
Assist with the following:
- Consult with at-risk patients in High Risk Antepartum, Labor and Delivery and NICU at HFWB and HFH to help process traumatic events such as long-term admissions, emergency cesarean deliveries, postpartum hemorrhage, severe preeclampsia, infant admissions, poor outcome, or loss.
- Ensure current evidence based-practice and clinical assessment protocols are being utilized by clinicians across the system
- Providing information to clinicians and patients regarding available mental health and support services within the community
- Implement ROSES as a virtual education and support tool, available to patients across the system
- Implement a supportive group or program for pregnancy and infant loss
- Creating a packet of tools and resources for new parents to take home upon discharge.
- Maintain contact with at risk patients during pregnancy and up to one year postpartum.
- Facilitate multidisciplinary care planning for at-risk perinatal patients or families with a poor prognosis (e.g. Trisomy 18, cardiac anomalies, PPROM)
