Opportunity to have work/life balance and learning within an academic healthcare institution.  

Work schedule is five - 8 hour shifts 8am-4:30 pm Thursday through Monday, every other weekend.

GENERAL SUMMARY: As part of the interdisciplinary palliative medicine team, provides comprehensive palliative medicine consultation to patients with serious illness in the inpatient setting. Provides comprehensive palliative care assessments, pain and symptom management, assistance with clarifying goals of care, and counseling to patients with serious illness and their families.  

ABILITIES/ SKILLS

1.     Performs comprehensive assessments of physical, psychological, and spiritual concerns in patients with serious illness and their families.

2.     Facilitates advance care planning conversations and assists with advance directive completion for patients with serious illness. 

3.     Manages pain and non-pain symptoms in patients with serious illness.

4.     Works in collaboration with the interdisciplinary team and the primary team to lead goals of care discussions for patients with serious illness.

5.     Recognizes and utilizes opportunities for health counseling with patients and families.  Works with the interdisciplinary team to meet the emotional needs of the patient and family/caregiver. 

6.     Provides education on concepts related to hospice and palliative care to other clinicians, healthcare administrators, and patients/families.

7.     Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition, care provided, symptom management changes/recommendations, and advance care planning/goals of care discussion documentation.

8.     For NPs: Currently certified as Hospice and Palliative Nurse (CHPN) or Advanced Certified Hospice and Palliative Nurse (ACHPN) from the Hospice and Palliative Nurses Association (HPNA) OR obtains certification within 2 years of hire.

9.     Participates in continuing medical education related to palliative medicine.

10.  Experience as a Palliative Medicine APP (NP or PA) preferred. Will consider applicants with experience as an APP and applicants with considerable nursing experience.

  • Provides inpatient and/or outpatient medical care.
  • Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs.
  • Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently.
  • Specific procedures are individually credentialed as indicated by training and experience.

CERTIFICATIONS/LICENSURES REQUIRED:

  • Requires Michigan state licensure and continued certification by the National Commission on Certification of Physician Assistant (NCCPA). -OR- Requires master's degree in nursing; licensure as a Certified Nurse Practitioner; licensure as a Registered Nurse
  • Current BLS or ACLS as required.
  • Current ATLS/PALS provider status as required by department.
  • The applicant graduated from an accredited PA or NP programs.
  • BLS certification or ACLS certification is a plus but will be required upon hire.
  • DEA licensure is a plus but will be required upon hire.
