  • 1. Performs comprehensive assessments of physical, psychological, and spiritual concerns in patients with serious illness and their families.
    2. Facilitates advance care planning conversations and assists with advance directive completion for patients with serious illness.
    3. Manages pain and non-pain symptoms in patients with serious illness.
    4. Works in collaboration with the interdisciplinary team and the primary team to lead goals of care discussions for patients with advanced illness.
    5. Recognizes and utilizes opportunities for health counseling with patients and families. Works in cooperation with the interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
    6. Works with palliative medicine RN to respond to patient phone calls and needs after hospital discharge and in between clinic visits.
    7. Provides education on concepts related to hospice and palliative care to other clinicians, healthcare administrators, and patients/families.
    8. Works with ambulatory palliative medicine team and leadership to increase referrals to palliative medicine clinic and for video visits, improve clinic operations, and participates in clinic quality improvement projects.
    9. Collaborates with home health palliative care team to most effectively care for clinic patients.
    10. Along with other palliative medicine team members, represents palliative medicine on Cancer Institute committees.
    11. Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition, care provided, symptom management changes/recommendations, and advance care planning/goals of care discussion documentation.
    12. For NPs: Possesses Certified Hospice and Palliative Nurse (CHPN) certification from the Hospice and Palliative Nurses Association (HPNA) or obtains certification within 2 years of hire, possess
    13. Participates in continuing medical education related to palliative medicine and oncology.


    Requires Master's Degree in Nursing; licensure as a Certified Nurse Practitioner; licensure as a Registered Nurse -OR- completion of an accredited Physician Assistant program and licensure in the state of Michigan as a Physician Assistant
  • BCLS Certification
  •  Certification in one of the following: ANA certification exam in a nursing specialty field; National Certification Corporation (NCC); National Certification Board of Pediatric Nurse Practitioners and Associates; and/or certification from Michigan State Board of Nursing as a Nurse Practitioner (NP) -OR- licensure in the state of Michigan as a Physician Assistant
  • A minimum of 1 year working as an APP is preferred
