GENERAL SUMMARY:

Under general supervision of the Home Infusion pharmacy manager and/or nursing supervisor, is responsible for assessing nutritional status and risk identification of enteral and parenteral nutrition patients and coordinates all aspects of patient home nutritional therapy.

 

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Processes the patient referral and creates electronic medical record in Home Infusion’s electronic medical record CPR+.
  • Answers incoming calls from other departments to resolve patient issues, e.g., billing problems, prescription refills, etc. Works with appropriate person/department to put appropriate action in place for resolution.
  • Observes and reports to the attending physician and other staff involved in the patient's care, the patient's reactions to and tolerance for the nutritional therapy and any changes in the patient's physical status or in the plan of care.
  • Works with marketing staff to identify potential growth opportunities for home enteral and parenteral nutrition therapies. Develops and maintains nutrition customer relationships. Works with marketing staff as the nutrition support content expert.

Enteral Nutrition

  • Develops and updates plan of care based on assessment of the patient's nutritional needs, physician's orders and assessment of the capability of the patient/caregiver to understand the patient's nutritional goals.
  • Calculates patient’s caloric intake based on clinical notes and chart review documents, determine order quantity and method of administration.
  • Performs follow up clinical assessment calls with patients/caregivers regarding patient condition and status, clarifies patient problems, determines urgency of patient problems, and responds with appropriate action to resolve patient problems.
  • Reviews patient documentation to ensure that accreditation, regulatory and reimbursement qualifying criteria are met and provides recommendations to referral sources and staff.
  • Requests and submits prior authorization requests to insurance company for enteral nutrition coverage.
  • Coordinates monthly prescription refill order with patient and insurance company.
  • Confirms delivery of enteral nutrition solutions and supplies with patient ensuring all items are received as ordered.
  • Processes patient discharge from home infusion services and arranges pick-up of all equipment including enteral pump; answers any questions regarding leftover formula or supplies.
  • Works with warehouse staff to ensure adequate levels of formula, based on estimated needs, availability of enteral supplies and equipment.

Total Parenteral Nutrition

  • Evaluates all new TPN therapy referrals and high risk patients for clinical safety, appropriateness and nutritional adequacy.
  • Evaluates TPN patients using reimbursement guidelines to ensure payment of services.
  • Assist staff involved in the patient's care, the patient and caregiver in understanding and following recommendations related to the plan of care.
  • Consults with the clinical team to understand the patient situation and provide nutrition options to present to the physician.


EDUCATION/EXPERIENCE:

 

  • Bachelor’s degree in Food/Nutrition or Dietetics and an approved dietetic internship required.
  • Registration with the Commission on Dietetic Registration is required; membership in the American Dietetic Association is preferred.
  • Minimum of three years of clinical experience is required; one year of home infusion or home health care experience preferred.
  • Knowledge of enteral formula conversions is required.
  • Knowledge of home infusion policies and procedures is preferred.
  • Knowledge of Medicare/Medicaid enteral nutrition requirements is preferred.
  • Knowledge of Insurance payer requirements and coverage is preferred.
  • Strong interpersonal skills are required to communicate effectively with medical personnel and staff; to obtain information from and explain dietetic restrictions to patients and families; and to design and conduct educational programs for Home Infusion team.
