Community Health Worker (Clinically Integrated Network) - Detroit Metro Area
This non-exempt full-time day Community Health Worker (CHW) position will work with the Clinically Integrated Network (CIN). The CIN is a group of healthcare providers - physicians, hospitals, clinics, labs, post-acute care specialist, etc. - who join together to improve health outcomes for patients and reduce the overall cost of care. The team members selected for this role will be working in clinics through the Detroit metro area to provide support to patients by conducting social needs screening and assessments. Responsibilities will also include conducting virtual and in-home visits in addition to community outreach.

GENERAL SUMMARY:
This role serves as a liaison, link or intermediary between health, social services and the community to facilitate access to services, and improve the quality of service delivery. This role also serves as an advocate for patients, a resource, and a vital part of the inter-professional healthcare team. Educates patients, engages and assists them in managing their healthcare needs. Provides non-clinical paraprofessional duties and is thoroughly familiar with the patients’ community and the resources available. Collaborates with healthcare staff, community based healthcare, social service and other providers.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Assist with patient retention, provision of referrals, and referral follow-up.
- Disseminate health information through one-to-one encounters with patients.
- Provide patients with information on HFHS classes, support groups and related services and assist with enrollment, if necessary.
- Conduct assessment activities related to social service and other ancillary needs related to the social determinants of health, and provide referral and follow-up related to these needs.
- Work closely with the patient care team to provide information on behalf of patient related needs, barriers and facilitators to accessing care, thus making it easier to design a tailored care plan for patients.
- Provide health education and coaching for patients.
EDUCATION/EXPERIENCE REQUIRED:
- High school diploma or G.E.D. equivalent.
- Previous experience as a community health worker, preferred.
- Previous experience conducting chronic disease education classes, preferred.
- Previous experience conducting home and/or clinic visits with patients and their primary care providers, preferred.
- Curriculum based education experience is helpful (i.e., parenting classes).
CERTIFICATIONS/LICENSURES REQUIRED:
Certification as a Community Health Worker or a willingness to become certified.
WORKING CONDITIONS:
Normal office environment with minimal exposure to noise, dust, or extreme temperatures. Travel to and from appointments located in patient homes. Must have a valid State of Michigan driver’s license and automobile insurance.
