Behavioral Health Triage Clinician (Psychotherapist) - Hybrid
Position Summary:
The Behavioral Health Triage Clinician is a licensed psychotherapist who provides brief behavioral health assessment, triage, and care coordination within an integrated care environment. This role serves as the clinical entry point for patients referred to behavioral health services and supports timely connection to the most appropriate level of care across our behavioral health service line offerings. This is a hybrid position.
Clinical Assessment and Triage
- Conduct brief behavioral health assessments to evaluate presenting concerns, symptom severity, and treatment needs
- Utilize standardized screening measures to support clinical decision making
- Respond to provider support requests for patients in acute distress or crisis, including real-time consultation and coordination of appropriate interventions
- Assess safety concerns and coordinate appropriate escalation when necessary
- Determine appropriate level of care and facilitate referrals within the behavioral health service line
- Manage urgent referrals, including prioritization, outreach, and timely disposition to ensure patient safety and access
Brief Clinical Intervention
- Provide brief, focused therapeutic interventions when clinically appropriate to support stabilization and engagement in care
- Utilize evidence-informed strategies such as motivational interviewing, psychoeducation, and problem-solving approaches
- Support patients in identifying initial goals and next steps for treatment
- Provide short-term follow-up and/or bridge appointments for higher-acuity or urgent patients when needed to ensure stabilization and successful linkage to ongoing care
Care Coordination and Navigation
- Coordinate referrals and support patients in connecting with appropriate behavioral health services
- Collaborate with providers to align care recommendations and treatment planning
- Assist patients in navigating behavioral health resources both within the health system and in the community
- Track and manage referral outcomes, including follow-up on high-priority or complex cases
Integrated Care Collaboration
- Partner with primary care and behavioral health providers to support coordinated patient care
- Communicate assessment findings and recommendations to referring providers
- Participate in interdisciplinary collaboration to support access and care coordination
- Serve as a clinical resource to providers for triage decision-making and level-of-care guidance
Group-Based Care Delivery
- Facilitate behavioral health groups as part of a rotating team schedule as needed
- Support patient engagement in group services as an alternative or adjunct to individual care
- Collaborate with team members to ensure alignment between triage, group placement, and ongoing care pathways
Required Qualifications
- Master’s degree from an accredited program in Social Work, Counseling, or Psychology
- Current, unrestricted clinical licensure in the state of Michigan (LMSW, LPC, or equivalent)
- Minimum of 1–2 years of clinical experience in behavioral health
- Experience conducting behavioral health assessments and developing treatment recommendations
- Strong clinical judgment related to risk assessment, crisis response, and level-of-care determination
- Ability to work within a fast-paced, team-based environment and manage competing priorities
- Excellent communication skills with ability to collaborate effectively with medical and behavioral health providers
- Proficiency in electronic medical record documentation
Preferred Qualifications
- Experience with triage, intake, or access-based behavioral health roles
- Experience in integrated care, primary care, or collaborative care settings
- Experience managing high-volume referrals and coordinating care across multiple services
- Familiarity with brief, evidence-based interventions (e.g., motivational interviewing, problem-solving therapy)
- Experience facilitating group-based behavioral health interventions
- Knowledge of community behavioral health resources and referral networks
- Experience supporting patients with high-acuity needs, including crisis stabilization and follow-up care
