GENERAL SUMMARY: 

Under the direction of Ambulatory Clinical Pharmacy Services, the Advanced Pharmacy Practice Provider (APPP) provides oversight and leadership for appropriate medication use within their region of the health system. He/she works closely with the medical and administrative leadership as well as the primary care providers to develop and implement strategies which continuously improve the quality of patient care at the best value for the patient and our health system. The APPP serves as an essential member of the ambulatory care team to achieve and maintain improved patient health outcomes, enhanced patient experience, reduced health care costs, and improved provider work life balance. The APPP practices at the top of licensure and with expanded scope of practice through a collaborative practice agreement (CPA). The APPP supports population health strategies and value-based care initiatives. He/she coordinates care with the provider, care team, and other pharmacy services across Henry Ford Health. The APPP participates in ongoing research, scholarship, quality initiatives, projects, committees, policy development, and formulary management strategies. He/she assists in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization. The APPP will be instrumental in guiding the further development of other pharmacists on the health care team.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  1. Provides comprehensive medication and disease state management activities via a Collaborative Practice Agreement (CPA) as an integral part of the primary care team. The expanded scope of practice includes, but is not limited to, initiating, modifying, discontinuing medications, ordering vaccines, ordering and interpretating medication related laboratory data, and designing and implementing medication, monitoring, and adherence plans.
  2. Collects and reviews patient medical and medication histories, assesses for signs and symptoms of disease progression, performs limited physical assessments and screening tests using standardized tools, facilitates ordering of medical equipment, and places referral for additional care, as necessary.
  3. Interviews the patient, family, caregiver, provides disease state and medication education, and provides care coordination and transition of care services.
  4. Using a data-driven approach to decision making, supports population health strategies and participates in value-based care initiatives.
  5. Provides exemplary services through the provision of pharmaceutical care to individual patients and the population. 
  6. Works collaboratively with leadership to provide expertise on relevant regional and system committees. 
  7. Provides education to patients, other health care professional, pharmacists, and students about optimal drug therapy. This may involve formal preceptorships of graduate and undergraduate students or pharmacists, and both formal and informal didactic presentations.
  8. Identifies, develops, and implements specific strategies to improve appropriate drug use and measure optimal patient care outcomes. This may include guideline and disease management approaches as well as formal application of scientific hypothesis testing. Serves as a positive role model and mentor for other pharmacists.
  9. Develops and maintains effective methods of providing drug information services to health professional on drug therapy, dosing, new medications, and research in collaboration with the Product Line.
  10. Has extensive clinical experience in various disease states including, but not limited to, diabetes, hypertension, dyslipidemia, asthma, COPD, congestive heart failure, coronary artery disease, chronic kidney disease, obesity, and/or various disease states within a specialty if practicing within a specialty clinic.

EDUCATION/EXPERIENCE REQUIRED:

  • Doctor of Pharmacy degree or equivalent experience with evidence of advanced clinical training and knowledge in patient care and applied therapeutics.
  • Required: PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care/Primary Care Specialty Residency and history of providing top-of-license clinical services under a collaborative practice agreement (equivalent clinical experience (preferably 3-5 years) in ambulatory care setting may be considered in lieu of PGY2 Residency).
  • Prior experience and knowledge or integrated health systems.
  • Prior experience working with relational databases.
  • Knowledge of Pharmacoeconomics and biostatistical applications in drug therapy decision making.
  • Exemplary oral and written communication skills as evidence with prior examples of effectiveness implementation strategies with a variety of system personnel and patients. Ability to lead work groups effectively.

CERTIFICATIONS/LICENSURES REQUIRED:

  • Registered Pharmacist in the State of Michigan.
  • Board Certified Ambulatory Care Pharmacist (BCACP) (Board Certified Pharmacotherapy Specialist (BCPS) considered) or eligible to take BCACP exam and pass within 1 year of hire.
