GENERAL SUMMARY: 
The ED Disposition Support (EDS) Supervisor manages the day-to-day operational performance of the new EDS program with minimal supervision. This program aims to ensure that patients are only admitted from the Emergency Room Department (ED) to an inpatient department or observation when clinically appropriate. Supervises a team of EDS Navigators in identifying and coordinating appropriate outpatient resources for patients in the ED who are being considered for admission. Participates in the identification and coordination of these resources, which include primary care, case management, community paramedicine, home care, skilled nursing, and transportation. In addition, the EDS Supervisor manages quality control for the community paramedicine component of the EDS program. Models positive collaboration with other members of a multi-disciplinary team and models a proactive and diligent approach to problem-solving. Supports team members through challenges and ambiguity. Performs all job duties and responsibilities in a courteous and customer-focused manner. 

PRINCIPAL DUTIES AND RESPONSIBILITIES: 
  • Manages day-to-day EDS operational performance with minimal supervision. Fosters/ensures an environment of respect and collaboration. 
  • Supports the professional development of direct reports and evaluates their performance. 
  • Collaborates with operational leaders, physicians, nursing and others to facilitate the maintenance of a high-performing service.
  • Supervises team of Navigators in identifying and coordinating appropriate outpatient resources for patients in the ED. Collaborates with a multi-disciplinary team. 
  • Communicates proactively and effectively with outpatient clinics, case management, an EMS provider, home care agencies, skilled nursing facilities, and other health care facilities and community organizations. 
  • Manages quality control for the community paramedicine component of the EDS Program. 
  • Completes reports on the community paramedicine intervention and submits the reports to state and medical control authorities. 
  • Maintains an awareness of federal, state and city regulations and Joint Commission or DNV standards. 
  • Understands all local and state Medical Control Authority regulations and protocols as they relate to EMS operations and scope of work. 
  • Leads process improvement activities designed to improve workflow and reduce risk. Inclusive of data collection, analysis and follow-up intervention activities.
  • Identifies and refers potential quality concerns to Quality Assessment/Risk Management/Legal Affairs. 
  • Performs other duties as assigned.
EDUCATION/EXPERIENCE REQUIRED: 
  • Bachelor Degree in healthcare or business related field, highly desirable. 
  • Three to five years of clinical experience, including experience leading special projects or initiatives. 
  • Ability to work collaboratively with cross-functional or cross-organizational teams. 
  • Mediation and conflict resolution skills. 
  • Excellent customer service and interpersonal skills, including the ability to communicate effectively with external customers and internal customers at all levels of the organization. 
  • Excellent written and verbal communication skills. 
  • Adaptable to changing environment. Creative and resourceful. 
  • Strong organizational, planning, and implementation skills with the ability to handle multiple project and timelines. 
  • Understanding of the needs of the diverse populations. 
  • Proficient in the use of computers, including Window and Microsoft Word. Microsoft Excel and Access preferred. 
CERTIFICATIONS/LICENSURES REQUIRED: 
  • Licensure as a Paramedic with a valid State of Michigan license. 
  • BLS certification
