Community Health Supervisor

About the Role
We are seeking a dynamic Community Health Supervisor to provide leadership, supervision, and strategic direction for community health initiatives. This role plays a critical part in advancing health improvement efforts through strong partnerships, effective project coordination, and evidence‑based approaches. The ideal candidate brings both public health expertise and proven leadership experience to support our mission of improving community well‑being.
The ideal candidate must have a master’s degree, a Public Health background, supervisor experience and experience in Grant management and budgeting.
Must have interest in or familiarity with substance use disorder (SUD) prevention models, treatment, or services.
Behavioral health background a plus.
Key Responsibilities
Strategic Leadership & Planning
- Contribute to the development and implementation of community health assessments and strategic action plans.
- Coordinate the local market Community Health Needs Assessment (CHNA) and Implementation Plan in partnership with system leaders, project teams, and community networks.
- Serve as a primary liaison to internal service lines and external community partners supporting CHNA activities and priorities.
Team Supervision & Management
- Provide daily supervision of Community Health staff, including recruitment, scheduling, performance management, and professional development.
- Monitor team processes and drive continuous improvement efforts to enhance efficiency and effectiveness.
Community Engagement & Partnerships
- Build, maintain, and lead strong community partnerships aligned with community health priorities.
- Represent the health system in local and regional initiatives, trainings, conferences, and events.
- Participate in advocacy, outreach, and marketing initiatives that support community health improvement plans.
Program Evaluation & Grants Management
- Design and implement program evaluation activities using both qualitative and quantitative methodologies.
- Lead grant research, writing, management, and reporting to advance community health efforts.
- Ensure accurate documentation of program activities, evaluation data, and budget expenditures in alignment with funder and organizational requirements.

EDUCATION/EXPERIENCE REQUIRED:
Master’s degree in public health or related field
- Three years of work experience in public health or related field, including experience supporting community health improvement projects and grants
- Formal leadership experience managing staff/team members, projects and deadlines
- Strong verbal and written communication and relationship-building skills
- Experience drafting successful grant proposals and managing grant awards
- Experience compiling and interpreting population health data/reports
- Experience applying critical thinking and excellent organizational skills to manage multiple tasks and projects
- Strong verbal and written communications with commitment to a healthy lifestyle as a model.
