GENERAL SUMMARY:

To provide psychosocial assessment planning, treatment and education for adult and geriatric patients and their families in collaboration with all members of the interdisciplinary group.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

·       Provides Medical Social Work Services in the place of residence to adult and geriatric populations and their families through complete assessment of the psychosocial needs affecting the patient/family and making appropriate written plans for care.

·       Participates in agency and system-wide continuous quality improvement activities to improve performance and patient outcomes.

·       Collaborates with leadership to develop and implement policies, procedures, CQI activities, reporting, training, and education to ensure organization-wide compliance with legal, ethical, contractual and/or regulatory standards (i.e., TJC, HCFA, OIG, Corporate Compliance, MIOSHA, etc.) 

·       Develops partnerships with all members of the health care team and exhibits behaviors that displays teamwork, professionalism, and respect for diversity. 

·       Participates in the orientation of new members of the health care team and exhibits behaviors that promote a positive learning experience.

·       Maintains a safe and risk-free environment for patients, their families/significant others, and other staff.

·       Creates a caring experience for both internal and external customers; is courteous and respectful, anticipates and responds to customer needs; and gives high priority to customer satisfaction.

·       Demonstrates personal accountability for own continuous growth and development and seeks out appropriate learning opportunities to enhance competency and improve performance.

EDUCATION REQUIRED:

·       Master’s Degree in Social Work accredited by the Council on Social Work Education

EXPERIENCE REQUIRED:

·       One (1) year of work experience in health care

·       One (1) year experience in Hospice preferred.

CERTIFICATIONS/LICENSURES REQUIRED:

·       Current Michigan Licensure – MSW

·       Current BLS certification
