The Post-Acute Skilled Nursing Facility (SNF) Collaborative Care Administrator designs, plans and implements a transformational approach to systematic to care coordination, which is competency central to fulfilling the Henry Ford Allegiance Health (HFAH) mission. Also responsible for leading, directing and fully deploying innovative initiatives that will transform models of care, which support the individualized needs of patients. Utilizing evidence-based approaches to improve care delivery, promote coordination of care, and improve outcomes. Operates under minimal direction and report to executive leadership. Regularly consults with leadership and gives recommendations for strategic planning, lead initiatives such as telemedicine, specialty care in the Skilled Nursing Facility (SNF), and EMR coordination with SNFs etc.

EDUCATION/EXPERIENCE REQUIRED:
Master’s Degree OR Graduate of an AMA approved Physician Assistant Program
Two (2) years of clinical experience
Post-acute care work experience
Ability to work weekends and an on-call schedule when necessary
Ability to perform rounding during high periods of utilization, as needed
Effective interpersonal communication skills required including verbal and written
Ability to problem solve
Effective organization and physical assessment skills
Must be able to work with diverse population of patient/family, caregivers, staff and physicians using reasonable tact and discretion
Must possess verbal, visual, and hearing abilities to comprehend and respond to written/verbal communication

CERTIFICATIONS/LICENSURES REQUIRED:
Registered Nurse License in the State of Michigan OR Nurse Practitioner Certification in the State of Michigan / Physician Assistant Licensure in the State of Michigan
Valid Michigan Driver’s License with good driving record, vehicle in good working condition and vehicle insurance required
