RN Case Management experience highly preferred.

EDUCATION/EXPERIENCE REQUIRED:

·       Have graduated from an accredited BSN program.

·       Possess RN licensure in the state of Michigan.

·       Possess a minimum of three (3) years of acute care/inpatient nursing experience.

·       Inpatient Case Management and/or discharge planning and care coordination experience strongly preferred.

·       Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem-solving skills and excel at planning.

·       Self-motivated and enjoy working in a fast-paced environment.
