GENERAL SUMMARY:

Under general supervision, helps patients and families adjust to personal and
environmental difficulties which predispose illness or interfere with their obtaining maximum benefits from medical care. Provides guidance and advice to medical and para-medical personnel regarding the impact of psychosocial factors on the treatment and progress of patients.To plan and coordinate services for patients/families in the discharge planning process, as well as to participate in Continuous Process Improvement activities, Customer Satisfaction measures and to support all departmental activities. 

PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Initiates and completes psychosocial assessments, interviewing patients, families and others as necessary in order to evaluate the effects of the illness or injury and to identify current or potential adjustment difficulties.
  • Based on assessments, develops a viable plan of intervention in collaboration with patients, patients' families and other parties as appropriate. 
  • Confers with supervisors and/or senior staff members on unusually difficult and/or complex cases.
  • Provides advice and counsel to patients and family members as appropriate. 
  • Refers patients needing individual or group therapy to senior departmental personnel, psychologists, psychiatrists or others.
  • Provides consultation to medical and para-medical staff regarding the impact of psychosocial factors on patients' adjustment to illness or injury.
  • Collaborating with patients, families and others, develops appropriate discharge plans for patients, ensuring that patients are aware of and referred to appropriate community resources.
  • As appropriate, serves as an advocate for patients, both within and outside of HFH.
  • Prepares other records and reports
  • Engages in case-finding activities as appropriate and when staffing levels permit.
  • Acquires and maintains a high level of professional competence in duties assigned.
  • Works with supervisor(s) in order to identify annual performance/development goals and the means by which these are to be achieved.
  • Performs other related duties as assigned.
EDUCATION/EXPERIENCE REQUIRED:
  • Masters degree in Social Work 
  • 3 to 4 years of medical social work experience 
  • ACSW certificate 
  • Behavioral Health experience, preferred
