Setting: Home Based-virtually rare occasion going into home.

Call/Weekends: Rotates on call for 1 week a month.

 

 

Position Overview:

 

Henry Ford Health is seeking an experienced, full-time Nurse Practitioner to join the Division of Hospice and Palliative Medicine. This role provides longitudinal palliative care for patients with serious illness, typically those with a prognosis of less than two years, with a manageable panel of approximately 200 patients. The NP will provide expert symptom management, advance care planning and goals-of-care discussions, coordinate transitions to hospice when appropriate, and collaborate with specialty providers while helping patients establish home-based primary care when needed. Working at the top of your license, you'll be part of a highly collaborative interdisciplinary team with daily interaction alongside home-based RNs and an excellent physician partner. The position offers the flexibility of a primarily remote schedule with travel to patient locations and monthly in-person interdisciplinary team meetings in Royal Oak (including training at the Josephine Ford Cancer Center). This is an excellent opportunity for an experienced NP seeking professional growth, continued education, and the chance to make a meaningful impact through Henry Ford Health's nationally recognized Hospice and Palliative Medicine program. Previous Nurse Practitioner experience is required; new graduates will not be considered.

Qualifications:

 

  • Certification as a Nurse Practitioner
  • At least 5 years’ experience with Palliative or Hospice
  • Specialty certification in hospice and palliative medicine (ACHPN for NP), preferred
  • 2 years’ experience in HPM, preferred
  • BLS, DEA, CALS

 

Education & Training

 

·         Nurse Practitioner (NP): Completion of a Nurse Practitioner program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC)

 

Licensure

·         Current licensure as a Nurse Practitioner, the State of Michigan, or in the process of applying (proof of application required)

 

Certification

·         NP: National certification by a certification body recognized by the State of Michigan (e.g., ANCC, AANP)

 

Required Certifications

  • Basic Life Support (BLS) – required
  • Advanced Cardiovascular Life Support (ACLS) – required (may be obtained after hire)

 

DEA / Controlled Substance Licensure

  • NP: DEA registration required through credentialing; NPs do not hold a Michigan CSL and will use their Michigan license number during the application process
