Nurse Navigator (RN) - Ambulatory Vascular - Critical Limb Salvage Program
This position will work with the Critical Limb Salvage Program. Vascular patients that are having difficulty navigating through a potential limb being amputated. Provide patient education and support. Navigating their care between various vascular surgeons, podiatrists, wound care, hyperbaric treatments, hospitalists, etc. This will involve face-to-face and phone interaction.
Location is still to be determined but in the Jackson area.
Full Time - 40 hours/week - 8 hour shifts - flexible start/end time. 8am - 5pm - no weekends/holidays
This is a salaried position.
Candidate Experience Preference: Minimum of 3 years RN inpatient/ambulatory/clinic experience. Cardiology / Vascular experience. Experience navigating patient care.

GENERAL SUMMARY: Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.
EDUCATION/EXPERIENCE REQUIRED:
Graduate from an accredited school of nursing required. Bachelor's Degree preferred. If ADN, must be willing to enroll in an accredited BSN program within 3 years of start date, obtain your BSN within 5 years of start date.
Two (2) years of nursing experience required.
Specialty experience preferred.
Maintains and demonstrates current clinical knowledge in area of specialty. Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills. Strong communication skills, written and verbal. Ability to prioritize and reprioritize quickly. Ability to develop collaborative relationships both internally and externally. Ability to work in teams and in a self-directed, autonomous manner. Able to delegate and coordinate efforts of the team/co-workers. Compassionate and empathetic. Understands Musculoskeletal Health specialties, pathways and procedures, goals of care, symptom management, quality of life, and evidence-based practice guidelines.
Demonstrates excellent coaching and mentoring skills. Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.
Computer skills and knowledge of Windows, Outlook and electronic medical record.
CERTIFICATIONS/LICENSURES REQUIRED: Current licensure by the Michigan State Board of Nursing as a Registered Nurse. Specialty-specific certification preferred and desired within two (2) years of hire as appropriate to job being filled.
NOTE: The Nurse Navigators for the Musculoskeletal Health Service Line already employed by HFHS orthopedic services have two (2) years to complete certification after entry into this job.
