This is an opportunity to practice with a skilled hospice interdisciplinary team to help improve quality of life for patients with life-limiting illness and support their families.  The position allows the NP to practice autonomously and serve as an attending provider for a caseload of hospice patients, while also having support from a hospice physician whenever needed. 

GENERAL SUMMARY:  Serves as a community hospice attending provider, with the supervision of a hospice physician.  Provides comprehensive hospice assessments, including symptom management, prognostic assessment, assistance with clarifying goals of care, and education and support around the dying process to hospice patients and their families.  

ABILITIES/ SKILLS

1.     In collaboration with the interdisciplinary team, performs a comprehensive history and physical examination to assess physical, psychological, and spiritual symptoms/concerns for hospice patients and their families.

2.     Collaborates with the interdisciplinary team to develop, create, implement, monitor, and evaluate patient treatment plans with a focus on symptom management for an assigned caseload of patients.

3.     Performs hospice face-to-face and symptom visits in-home and virtually.

4.     Participates in hospice quality management/improvement activities.

5.     Supports and contributes to the development of learners including nurse practitioner students medical students, residents and/or fellows.

6.     Recognizes and utilizes opportunities for health counseling with patients and families.  Works in cooperation with the interdisciplinary team to meet the emotional needs of the patient and family/caregiver. 

7.     Provides education on concepts related to hospice care to other clinicians, healthcare administrators, and patients/families.

8.     Demonstrates skill in empathic and honest conversations about impending death and hospice care with patients, families, physicians and community groups.

9.     Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition, care provided, treat, and advance care planning/goals of care discussion documentation.

10.  Possesses Certified Hospice and Palliative Nurse (CHPN) certification from the Hospice and Palliative Nurses Association (HPNA) or obtains certification within 2 years of hire.

11.  Enhances professional growth and development through participation in education programs in hospice and palliative medicine, reviewing current literature, in-service meetings, and workshops. May participate in institutional committees and professional activities at local, state and national levels.

Functions as a team leader, acting as a resource to support the clinical practice.

  • Provides inpatient and/or outpatient medical care with the supervision but not necessarily the presence of a qualified physician member of the medical staff.
  • Takes patient history, performs physical examination, orders and interprets tests, makes diagnoses and institutes appropriate treatment programs.
  • Performs nursing functions independently.
  • Works in collaboration with staff physicians, house officers, nurses and other members of the medical care team to ensure that patient outcomes are achieved in a safe and efficient manner.
  • Specific procedures are individually credentialed as indicated by training and experience. 

EDUCATION & EXPERIENCE:

  • Requires master's degree in nursing. 
  • five (5) years of NP experience to become a Leader. 
  • licensure as a Certified Nurse Practitioner; licensure as a Registered Nurse. 
  • BCLS certification; and certification in one of the following: ANA certification exam in a nursing specialty field, National Certification Corporation (NCC), National Certification Board of Pediatric Nurse Practitioners and Associates, and/or Certification from Michigan State Board of Nursing as a Nurse Practitioner (NP).
