General Summary:

  • Assesses and identifies patient needs in accordance with the hospice concept of care. 
  •  Coordinates with interdisciplinary team to provide complete patient/family care and referral.
  • Provides pain management and symptom control.
  • Maintains the continuity of care by ensuring patient needs are met.

EDUCATION & EXPERIENCE:

  •  Requires graduation from an accredited school of nursing.
  •  Requires current licensure in the State of Michigan as a Registered Nurse. Hospice certification preferred.
  • Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
  • Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program.
  •  Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families.
  •  Requires some on-call.
  • Requires a valid drivers' license in the State of Michigan and an acceptable driving record.
  • Background in oncology nursing or previous hospice experience preferred.
