Coordinator-Population Health (days full time) One Ford Place
- Bachelor’s degree (preferably in one of the following disciplines; Health Administration, Health Science, Community Development/Outreach, Public Health or other health related field) or five (5) years of experience with an Associate’s Degree.
- Three (3) years of work experience in population health, community outreach and/or health coaching (or similar experience in a health related field).
- Customer service experience is strongly preferred.
- Experience using personal computers.
- Proficiency with Windows-based software, including spreadsheet (Excel), relational database (Access) and graphics software (PowerPoint).
- Experience or familiarity with community support services preferred.
- Familiarity with HEDIS preferred.
- Familiarity with Population Health preferred.
- Excellent interpersonal skills.
- Good organizational and communication (verbal and written) skills.
- Demonstrated ability to work in team setting.
- Demonstrated ability to work under limited supervision.
- Shows the ability to resolve conflicts with patients and overcome their objections about their health and behaviors.
- Good quantitative and analytical skills.
- Demonstrates a commitment to patients and a willingness to provide care.
- Aptitude for computer-based systems.
- Ability to analyze, evaluate and use critical thinking to make decisions and problem solve.
- Ability and willingness to self-motivate, self-manage, remain flexible and be agreeable to a change in processes when necessary to improve effectiveness.
- Must possess a valid Michigan driver's license and maintain a driving record that would qualify for a Michigan chauffeur's license endorsement.
