PRINCIPAL DUTIES AND RESPONSIBILITIES:

  •  Provides patients with reproductive and sexual health materials, educates patients on contraceptive methods and associated reproductive health topics, screens patients for other women’s health needs, and provides patient referrals to reproductive health and social services partners.

  • Provides health information through one-to-one encounters with patients in the Emergency Department and follow-up phone calls, texts, emails, or social media direct messages.

  • Provides patients with information on HFHS classes, support groups, and related services and assist with enrollment, if necessary.

  • Conducts assessment activities related to targeted reproductive and sexual health prevention, social services, and other needs related to the social determinants of health.

  • Provides referrals and follow-up related to identified reproductive and sexual health prevention, social services, and other needs related to the social determinants of health.

  • Works closely with the patient care team to provide information on reproductive and sexual health and barriers and facilitators to accessing care, thus making it easier to design a tailored care plan for patient.

  • Provides reproductive and sexual health education, decision-support, and coaching for patients.

 

 

 

EDUCATION, EXPERIENCE, AND SKILLS QUALIFICATIONS:

  • High school diploma or G.E.D. equivalent required.
  • Significant experience and deep knowledge of Detroit required.
  • Must meet or exceed core customer service responsibilities, standards, and behaviors as outlined in the HFHS’ Customer Service Policy and summarized below (required):
  • Communication,Understanding, Sensitivity Teamwork, Ownership,Motivation, Excellence, and Respect
  • Must meet or exceed the following skills (required) Strong organization, time management, attention to detail, and problem-solving skills.
  •  Ability to work independently and remotely, including use of phone or web meetings for team management.
  • Respects diversity without judgment and works to create an inclusive environment for all.
  • Flexibility and adaptability to project needs and work environment.
  • Excellent interpersonal, written, and oral communication skills.
  • Ability to receive constructive feedback to improve job performance and team success.
  • Proficiency with Microsoft Word and email.
  • Must be able to practice the customer service skills as provided through on-going training and in-services.
  • Previous experience as a community health worker, health educator, or similar preferred, but not necessary.
  • Previous experience providing contraception or related health education, preferred, but not necessary.
  • Previous experience working in a fast-paced healthcare setting such as the Emergency Department, preferred, but not necessary.
  • Should feel comfortable working in a "chaotic work setting".

CERTIFICATIONS/LICENSURES REQUIRED:

All trainings and certifications will be provided by employer if not already attained, including:

Michigan Community Health Worker Alliance (MICHWA) Community Health Worker certificate

  • Contraception counseling and family planning training
  • Research training necessary for this role
  • Emergency Department and healthcare navigation training
