Case Management Associate - Behavioral Health - Full Time - Days - Allegiance Health - Jackson, MI
EDUCATION/EXPERIENCE REQUIRED:
- Bachelor’s degree required
- Experience working with patients who have co-occurring mental health, substance use disorder, and physical health concerns.
- Discharge planning and behavioral health case management required
- Exceptional computer and analytical skills.
- Ability to establish priorities and coordinate work activities among multiple requests
- Must be capable of standing, walking, stooping, bending and lifting up to 25 pounds.
PREFERRED QUALIFICATIONS:
Bachelor of Social Worker license in the State of Michigan
Minimum of two years of experience working with behavioral health patients in an ambulatory and/or community health setting preferred.
Working knowledge/experience in utilization management, managed care, and payer issues is preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
State of Michigan Social Worker license if applicable.
