This position will be a blend of remote work and weekly travel to hospitals, clinics and post acute care locations throughout southeast Michigan, Jackson, and Flint.

GENERAL SUMMARY:

The POST-ACUTE NETWORK (PAN) Program Manager (PM) reflects the mission, vision, and values of Henry Ford Health, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

The PAN Program Manager is responsible for overseeing and managing the Post-Acute Network for providers such as skilled nursing facilities, home health care, and hospice. The PAN PM will lead various business and system-related projects of a complex nature within the organization, including all regional strategic and operational initiatives for the Post-Acute Network providers with an emphasis on continuous quality improvement. This position supports the Director in achieving their strategic goals for the post-acute networks by assisting in the timely and cost-effective delivery of all projects in a professional and customer-focused manner. The PAN PM will be part of the Henry Ford Health Post-Acute Department team and will provide leadership in developing a high-performing network that incorporates leadership principles and vision in performing operational functions.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Utilizes expertise in ideation, facilitation, strategic planning, and relationship management to coordinate multiple teams within the organization and collaborate with external partners. This effort aims to establish an overarching vision, align strategies, define scope, create a roadmap, develop a business case, and formulate a cohesive plan for the execution of the assigned program.
  • Manages post-acute network metrics, milestones, roadmap, schedule, and reporting to ensure strategic and operational initiatives meet Henry Ford’s quality, strategic goals, and clinical outcomes for its population and Populance customers.
  • Partners with many multidisciplinary teams, including technical teams, care coordination, hospital leaders, vendor teams, and post-acute providers, to ensure program goals and outcomes are achieved.
  • Ensures quality care in the post-acute network, focusing on outcomes like readmissions, length of stay, timely care, and network usage.
  • Collaborates with Henry Ford Health and post-acute partners to implement education, care coordination, care pathways, IT, and data for effective patient care.
  • Collaborates with care coordination teams to ensure Henry Ford Health patients receive quality post-acute care services.
  • Maintains communication with the post-acute network and system partners to share data and quality metrics for continuous improvement, including site visits.
  • Serves as the main communication point with cross-functional stakeholders, including post-acute providers, hospitals, care coordination teams, and business and IT/analytics partners for the assigned program; maintains partnerships and ensures timely and accurate communication of program updates.
  • Evaluates opportunities for and advises senior leadership on partnerships, and collaboration opportunities in the post-acute space.
  • Lead activities including project proposal/charter definition, resource allocation and management, information distribution and meeting facilitation as well as a comprehensive planning effort necessary to establish, measure, enhance and report organizational management performance.
  • Works closely with all relevant stakeholders to set program goals, clearly define operational dependencies, develop appropriate performance standards, metrics and reporting tools required for successful program design.
  • Demonstrate the ability to deal with the changing environment and to lead/facilitate and manage change efficiently and effectively where appropriate.
  • Be responsible for understanding post-acute provider contracts with Henry Ford Health to ensure post-acute provider compliance to project schedules, contractual agreements, and compliance with Henry Ford Health standards.
  • Develops and maintains productive and cordial relationships with clients and team members, serving as a link between multiple parties and entities; effectively manages project risks, issues and problems that arise within the program and recommend mitigation strategies, as needed, to achieve the desired results.
  • Analyzes large data sets to identify program opportunities and provides performance updates for cross-departmental initiatives and executive oversight.
  • Utilizes innovative thinking to create solutions that address challenges and fulfill project requirements and expectations.
  • Other duties as assigned.

EDUCATION/EXPERIENCE REQUIRED:

  • Bachelor’s degree in healthcare related field such as BSW, BSN, health management/administration, Project Management, and/or business administration 
    degree.
  • Minimum 3-5 years of healthcare related experience in healthcare delivery, project management, and running large projects of a complex nature, required.
  • Minimum three (3) years of experience in project management or analytics-oriented roles, focused on delivering strategic insights to customers/clients.
  • Two (2) years of experience using data to drive business decisions, preferred.
  • Strong knowledge of Microsoft Word, Excel, Power Point, Project Management.
  • Previous clinical experience delivering patient care preferred.
  • Excellent verbal communication and written documentation skills.
  • Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization.
  • Strong problem-solving, analytical, and decision-making skills.
  • Strong organizational, planning and implementation skills.
  • Strong presentation skills.
  • Strong sense of compassion with the ability to successfully advocate for patients and their families.

CERTIFICATIONS/LICENSURES REQUIRED:

  • Project Management Institute Certification, preferred.

PHYSICAL DEMANDS/WORKING CONDITIONS:

  • Requires 80% local travel to post-acute care facilities within the market area.
  • Unpredictable environment with wide variations in the types of patients and acuity levels, varying conditions regarding temperature control and potential exposure to hazardous materials.
  • Normal hazards associated with healthcare environment.
