Director of Case Management - Henry Ford Hospital
GENERAL SUMMARY:
Reporting to the Vice President of Acute Case Management, the Director of Case Management is responsible for the development, implementation, and/or monitoring of the case management department for Henry Ford Hospital. Serves as a primary source to coordinate and communicate with administration and the treatment team the financial management, interdisciplinary patient care service delivery and discharge planning processes. Works closely with senior physician and operational leadership to assure that the case management program complies with standard treatment protocols and lengths of stay.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Leads site strategic/action planning, ensuring alignment of all Case management strategic initiatives to system/hospital goals and objectives.
- Leads initiatives, meetings, communication among the Site Case Managers and presents at leadership forums as needed.
- Designs and implements systematic, standardized processes spanning region that enhance operational effectiveness and the patient experience in the hospital setting in alignment with broader system strategy and goals.
- Develops a process and leads a team that integrates information related to performance improvement priorities, measures region performance and advises leadership of opportunities. Identifies targeted opportunities for improvement then develops and implements action plans.
- Performance coaching and counseling to assigned team members, interviews job applicants for departmental vacancies, conducts performance reviews, recommends pay increases, and recommends discipline and termination of assigned personnel as warranted.
- Analyze physician utilization patterns, comparing to national and individual hospital standards.
- Maintain a working relationship with local, state, and federal agencies, recognizing the hospital’s position in the community and its need for cooperation and assistance from such services.
- Responsible for the ongoing development, formalization, and administration of the inpatient case management program.
- Collaborates with senior leadership regarding patient flow, census management, patient care outcomes, and financial performance indicators.
- This position is also responsible for relationship building, as defined through targeted goals of the business plan, with insurance companies, self-insured employers, case management firms, and/or other health care networks.
EDUCATION/EXPERIENCE REQUIRED:
- Master’s degree in nursing or a Master’s degree of Social Work, required.
- Minimum three (3) years of clinical experience, required.
- Minimum three (3) years leading teams, required.
- Excellent verbal communication and written documentation skills
- Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization.
- Strong problem-solving, analytical and decision-making skills.
- Strong computer skills
- Experience in discharge planning, home health care, rehabilitative medicine, community health or managed care preferred.
- Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare and Medicaid regulations and case management principles.
- Knowledge of medical ethics and legal implications related to case management.
- Understanding of social determinants of health and their impact on a patient’s wellbeing.
- Well versed in facilitating community resources to meet the needs of diverse populations.
- Strong organizational, planning and implementation skills with the ability to handle multiple complex patient needs simultaneously.
- Strong sense of compassion with the ability to successfully advocate for patients and their families.
CERTIFICATIONS/LICENSURES REQUIRED:
- Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
- Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) preferred.
