GENERAL SUMMARY:

Reporting to the director or above responsible for the day-to-day management of a Henry Ford Care at Home assigned office in accordance with divisional standards and policies. Manages the activities of assigned personnel. Participates in the formulation and implementation of policies and procedures relating to a Henry Ford Care at Home division. Evaluates, assesses and recommends improvements in services to the director. 

EDUCATION/EXPERIENCE: 

  • Requires one of the following: 

     

    • Medical Social Worker - Master's Degree (LMSW)

       

    • Registered Nurse, Associates Degree, Bachelor's Degree preferred from an Accredited School of Nursing

       

    • Physical Therapist 

      Bachelor's Degree (Bachelor's degree with 5 years of experience OR Master's/Doctorate degree acquired from a Commission on Accreditation of Physical Therapy (CAPTE) accredited program.). 

       

    • Occupational Therapist - Bachelor's Degree (From an Accreditation Council for Occupational Therapy Education (ACOTE) accredited program). 

    • Speech Language Pathologist - Master's Degree (Master's Degree in Speech Language Pathology from an accredited graduate program; successful completion of the National Praxis Examination; Successful completion of the Clinical Fellowship year.). 

  • Requires a minimum of (3) years of progressively responsible health care experience, one (1) year of which should be in care at home arena; previous supervisory experience is preferred. 

  • Analytical ability required to evaluate the effectiveness of policies and procedure and make recommendations for improvement. 

  • Strong interpersonal and communication skills required to provide guidance and supervision to assigned leadership, professional, support and clerical staff and to interact with all levels and types of internal and external customers. 

  • May need to lift, carry, bend, stoop or be otherwise physically active in the normal course of job duties. 

     

     

    CERTIFICATIONS/LICENSURES REQUIRED: 

     

  • Current licensure/certification by the State of Michigan as a therapist, social worker, or registered nurse.
